Inpatient Rehab & LTCH Index is compiled from public records, not from who paid to be listed.

We read the CMS Provider Data Catalog general-information files for inpatient rehabilitation facilities and long-term care hospitals. Each row becomes one listing and gets the Medicare-certified badge. We keep rows with a facility name, city and U.S. state or territory.
We show only what the CMS file contains: certification number (CCN), ownership type, beds (LTCH file only), the Medicare certification date, county and phone. We do not publish quality scores, inspection findings or patient outcomes; each profile points to Medicare Care Compare for those.
Order within a city: longest Medicare-certified first, then alphabetical. Featured listings appear in a separate, labelled box and never change that order. Placement is a flat monthly fee; we never charge per referral or patient.
What a badge means
Medicare-certified: the CMS Provider Data Catalog lists the facility as a Medicare-certified inpatient rehabilitation facility or long-term care hospital. The badge means it is in that file; it is not a quality rating. We only publish positive matches. A listing without a badge is not an accusation — most gaps are name variations between registries.

Data was last refreshed on 2026-10-02. CMS updates these files regularly; we re-load them regularly.
